I know I am not adding to what you already know, but the mix of quality components suggests it is a quality bike that some previous owner thought was worth upgrading.

Unfortunately, it subsequently met with some fixed object. Someone went as far as replacing the front wheel with a V-section wheel, but did not bother to straighten the fork, and then trashed the whole thing when it didn't track straight. The fork may be too far gone to straighten, but I would give it a try before seeking out a replacement - assuming frame is not bent.
