Another exception not yet mentioned, bicycle travel on Interstates in some parts of the American West. Yes Virginia, bicycles are allowed on Interstates in parts of California, Nevada, Arizona, Washington & Oregon - possibly more - where the Interstate is the only road.
I wouldn't even want to consider riding someplace other than on the shoulder.

restrict them to the shoulders.
