The all chrome fork is not original, suggesting that the original one must have experienced damage, necessitating replacement. As for what model of bike, the pictures you offer, though big and clear, do not depict the frame features that would help to determine quality level and, perhaps, model.

Were you to take good, clear pictures of the drive side drop-out, the bottom bracket from the left side and fork drops, it would be easier to get an idea of what the bike is.
