Watch out for glare ice all the way across a road -- I should have taken the alternate route and avoided it, but I didn't; the result was 'rear wheel slideout, foot down to catch fall, SNAP! of left anklebone as it overstressed to maintain. That was last December.

'08 was a REAL no-brainer -- as in I left my brains elsewhere....

New winter gloves, too thick for tactile feedback on the brake levers, a too-soft fork spring (I ride MTB), and a too-stout front brake conspired to dead-sailor me into the street, where I executed a 'perfect' 2-point landing... on my helmet and right shoulder. Turned a single-piece collarbone into a toddler puzzle (6 pieces). All of these have since been fixed, never to happen again. I think I wore those gloves about 3x last winter, every time when I was catching the bus.
