Epic saga! My patch did not fail. There was another hole, suspiciously close to the hole I patched. Very suspicious. The LBS couldn't find anything in the tire, and neither could I, so they replaced the tube.
Got the back back in the office and just for the heck of it, really carefully examined the tread. I saw a little nick in one of the knobs and pried it open with a fingernail. Something shiny was in there!

Stupidly, I started picking at it with my Leatherman to see if I could dig it out. Guess what happened next? "Psssssssssssssssssssssssssssssss......"
Back to the LBS. He picked out a bunch of broken glass bits and blew through the hole with compressed air. Hopefully that's the end of my bike problems.
It beats getting a flat on the ride home!
Those little shiny pieces in miniscule holes in the tread were the bane of my existence for a couple months with increasing frequency. Finally, I eventually decided that flats at least twice a week and approx. 3,000 miles on my cheapy tires, it was time to replace my tires. New tires on for three weeks or so and not a sinlge issue. Ah... piece of mind.
