Unique fit but I can't be the only one...

Here is my issue:

About a month ago, I got cycling shoes and put road pedals on my bike that I bought last July. On the trainer when I was being fit and just riding casually around the neighborhood (no more than 9mph) everything was fine. I then went out for a longer, faster ride this weekend and within the first 4 miles of my ride my left knee hurt on the sides. My left hip is rotated forward slightly. Before I put on the cycling pedals, my left foot angled out to the left when I put my foot in the cage and my heel would hit the crank bar. I assume this is the correct position for my foot since it naturally seems to sit in the cage that way. It was the first time I've ever ridden with cages and the first time my heel hit the crank bar in the 23 years I've known how to ride a bike. I assume that it is because of the restriction from the cage not allowing my put to sit far enough on the outside of my pedal.

My questions are the following:

1. Does that sound right that my foot might angle out that way?

2. Do they make pedal extenders so that if I have to angle the cleat enough my heal won't hit the crank bar?

3. Is it even possible to angle the cleat that much?
