I typically carry a tire lever, Co2 pump w/ two cartridges, a spare tube, patch kit, 15mm stubby wrench and a multi-tool.

However, the last time I got a flat, I fumbled with my Co2 inflator while installing the cartridge, dropped it and it went down a damn storm drain. It was a long walk home in the middle of the night.
