looks like a chorus 102 from here but ive been wrong before. well, it could be a chorus 111 for a triple as well.
chorus/record double = 102
chorus triple/record = 111 asymmetric
centaur/daytona/veloce double = 111 symmetric
centaur/daytona/veloce triple = 111 symmetric OR 115.5 depending on frame
