It would only occur in a couple of places if that were the case and it seems as though he is describing that this occurs throughout the entire chain's travel.

Good point about troubleshooting as much as you can - swap chains, swap cassettes, etc....rule out everything that you can. If you swap something and the problem doesn't go away then it wasn't that component.
