I tore apart the rear wheel on my Specialized Hardrock. The rear axle threads underneath where the freehub sits are completely deformed. What would cause this to happen? Everything else looks fine, and there isn't any excessive wear on the cones or anything to indicate a ton of grit is in there.

Based on the bolded part it almost sounds like you're confusing a freehub setup with a freewheel setup. On a freehub setup it's not possible for anything to get into the axle where you're describing. But on a freewheel setup it's possible that a pebble could get in there under unusual circumstances.
