Based on the bolded part it almost sounds like you're confusing a freehub setup with a freewheel setup. On a freehub setup it's not possible for anything to get into the axle where you're describing. But on a freewheel setup it's possible that a pebble could get in there under unusual circumstances.

Check out the two types at www.parktool.com/repair and confirm which style you have.
