Would creating a document something along the lines of:

I John Doe do hereby grant EMTs/ER techs/doctors/nurses permission to access my medical records in the event of an emergency and I am unable to do so.

Than not only signing it but having it notarized as well. Satisfy the HIPAA requirements?
