For insurance purposes - what level of proof would insurance companies require (do I need to get as many receipts as possible from the original owner) and also what would you insure it for, the price I got it for, or the new value, or what would you say the replacement value was?

Thanks!
Never ask more than the new price to be able to buy insurance. Get as much documentation as possible to support the new price.

The best insurance deal would be full replacement due to damage or theft regardless of age of the bike so that whatever happens you get a new bike of equal value.
