Here's a bike I set up for my boss, who has back problems:



$20 stem extender, $20 high rise stem, and $15 steel north road style bars. The riding position is completely upright. Handlebar mount shifters are probably next.

The Nitto products are great, but there are cheaper, functional alternatives.
