1)What helmet do you use?
Trek It's blue with lot's of vents and a visor.

2) What did you pay for it?
Got it with the bike off CL... $200 IIRC

3) And when/what will make you decide to replace it?
When it saves my noggin, get's lost or STOLEN! :-)
