Bingo. I had the same problem with an Ultegra triple FD and a 48T large ring.
The FD was so far above the large ring that middle->large shifts were nearly impossible.
I went to the IRD Alpina-D, and the cage was shorter, enabling me to put it right down to the chainring, giving me perfect shifting.
The Ultegra FD is now on the GF's bike where it works great with a 50T large ring.
