The are closely monitored in matching nutrition with their workouts (about 5 hours a day) in order to get the optimum weight loss. This is a complex relationship...... I remember a show a couple years ago where on guy didn't lose or gained weight and they figured out it was because he cut more calories in his diet than called for.
I get the why can i thing and just think wham let me lose it fast.....but losing weight consistently by make changes you can live with forever....is what keeps it off.
Biggest Loser is not "the norm" as you said the contestants workout 5-10 hours a day; they have RD's, MD's and XYZ's giving them the "right diet" and the "right way to workout"! They also do not weight-in on every 7 day period according to some of the contestants from past seasons.
As for reality; I can tell you the first 50-75 lbs comes off 2, 3 and 5 pounds in a week then the next 25-50 come off a pound or two a week after that the fight is on! Here is part of my Sig line from another forum; I started my lifestyle change 9/1/09 at 290#

I walked an hour or two most days or did an hour on the tredmill or 14 miles on the bike till this past summer then it was an ave of 18 or so mile.
