Once the cogs and chain get really worn out, the chain will slide up the teeth and skip. I doubt this is your problem as you describe the problem is happening in all gears. In any case, I doubt it is a bearing or freewheel problem.

I wish I could provide more. One thing I could mention is that I used to have another bike quite awhile ago that was starting to do the same thing. It became more and more frequent until finally the chain broke and then I ended up getting rid of the bike. It was in bad shape anyway. I always assumed the chain broke from all that violent jerking that happens when I'm pedaling and suddenly it seems to freewheel for a split second and then suddenly catch again. I can see how that would put a strain on things. I'm now staying off my current bike until I can figure out how to fix it so I don't make things worse. It's not very old, and I had been taking care of it, having learned from the past.
