For a Kelly-like setup you'd need 2x bands, grind the unneeded boss off each one. As for indexing, since the right shifter is on the left drop and the left shifter is on the right drop you'd have to criss-cross cables (or just get used to rear shifting on the left).
So yeah, if you can get a couple of stem shifter bands with correct bosses for the shifters and some 3x8 downtube shifters then that would be one way to go. Seems like the lack of thumb clearance would be a bummer, though.
When I said stem shifters, I meant just give up on the dream of shifting on the bars and shift on the stems. Cheaper than most new bar end options. Thumbies or your mods of stem shifters, I'm not sure if you could adjust the start so thumb clearance isn't a problem or not.
