Ah, that explains it... All garments based on Chest size, make an assumption about waist size, but if your chest to waist ratio is more then the assumption, then it's not going to fit.
Even the chest was a bit more snug than I prefer, but at least it didn't look (quite) painted-on. I can't say as much for the waist....
