I don't know where the factory that makes 3T sources their Ti bolts but one of the problem manufacturing in Asia is consistantancy in the quality of materials, esp metals.

Since they buy std Ti bolts by the tens of thousands, if a thousand bad bolts get into a lot is it impossible to trace them. They are just out there waiting to fail.

If what the OP (and others) are describing is accurate then clearly that is the problem here.
