The wheel needs to be retensioned or the rim is bent. Your "going out of true" is either because of a tension imbalance, the wheel is way undersized for your weight, or you bent the rim.

The front wheel - roughly - should have equal tensions on all of the spokes. Pluck yours. If by plucking them it sounds like you're playing a sound instead of tuning an instrument...then you're out of balance.
