Because I experience that a lot.
I'm SURE that's part of the issue....but I was a rock climber since high school, and the strong hands are paramount in that endeavor. I quit about nine years ago so hand strength may be part of the equation.
I think B-Girl hit the nail on the head. The calipers are so squeezed together that the cable exits the adjusting barrel at a noticable angle.
