This sounds pretty close to my situation. What you refer to as knots the physio calls trigger points. He gets rid of them in 4 seconds with dry needling. But they will always come back if the actual problem is still there.
Did you also find the pain (or really discomfort) was alleviated by standing up in the saddle?

No I didn't notice if standing helped at all. In fact, cycling in general seemed to help but it was afterwards that brought on the pain. That's when I started to investigate the possibility of muscle strain & spasming.
When you say the doctor "dry needles", is he pressing on a specific location really hard?
