I think at least two apply in Arizona:

Mt. Graham:
Distance - 20 mi
Peak elevation - 9,000ft
Elevation gain - 5,689ft
Gradient - 6%

Mt. Lemmon:
Distance - 27 miles
Peak elevation - 8,400ft (ski area) 9,000ft observatory.
Start: ~3000ft
Elevation Gain - ~6000 ft
Av Gradient - 5%

Finishing at 9000 ft. takes the breath away.
