There really isn't a "standard way". On the TA we used about 1/3 of a bed sheet that we bought and ripped into three pieces. Some people sew up an envelope to cover their sleeping pad. Since I started packing lighter and lighter I have gone to just wearing a thin tech tee so my skin wasn't directly on the mattress. Any of the three approaches can work fine and there are probably others as well.
