This is most likely caused by improper installation the previous time. The tube gets caught in between the tyre and rim (not fully enclosed by tyre). The pressure eventually pushes the tube through to the outside and you end up with a classic star-shaped or slit hole in the tube.

When installing a new tube, after it's all back together, let out most of the air. Then pull the tyre sideways and look into the gap between the tyre and rim, no tube should be visible. Then move over 6" and pull the tyre over again and inspect. Repeat until you've inspected the entire circumference of the tyre. Then repeat for the other side of the tyre. Only when you've completely inspected the edge of the tyre on both sides and confirmed that no tube is visible, then you would pump it up to full operating pressure.
