Very strange. Am I missing something, or is this a common thing. The tire was a two-year old Kenda nubby 27 x 1 3/8 and the tube was three weeks old!
Probably a weak spot in the tire, as FB said. I had a similar incident a few years ago, but the bike was just sitting out in the sun (and it was 104° that day). It was a rest day on a supported tour, so my wife and I were walking around town. When we got back to camp, 5 different people came up and said "your tire blew out". The damage was like you describe: smallish hole in the tire, burst tube.
Tire blowouts aren't as common as pinch blowouts, but they happen. Even worse is a rim blowout:
