Sheldon explain adjusting the hub here:
http://sheldonbrown.com/cone-adjustment.html but if it's that far out of adjustment you may need to overhaul the hub (link @ bottom of same page) to look for damage. Not out of the realm of possibility that you would find a broken axle if you have not removed it from the frame yet.
