I can't think of any adjustable stem that'd let you get your bars both 100 mm out and up. An adjustable stem set high would give you a decent rise, but would move the bars closer.

My suggestion would be to get a steerer tube extender, like this perhaps:http://www.nashbar.com/bikes/Product...39_-1___202438

Then start trying out various stems
