It's more common than you'd think for problems to manifest while riding but not on the stand.
If turning out the high limit screw all the way doesn't let it shift into the smallest cog, you may need a little more slack in the derailleur cable.
No it isn't a cable problem as the derailleur have no problems moving to the limit i set.
