The hose is easy. Any hardware or auto parts store sells small diameter air compressor or high pressure fuel hose by the foot so get as much length as you want. The heads are available from several sources in both presta only and Schrader/Presta combinations. Here is Amazon's listing for both hose/head assemblies and heads only:
