The hose is easy. Any hardware or auto parts store sells small diameter air compressor or high pressure fuel hose by the foot so get as much length as you want. The heads are available from several sources in both presta only and Schrader/Presta combinations. Here is Amazon's listing for both hose/head assemblies and heads only:

As I recall on my pump, the hose threaded INTO the base of the pump, or the fitting there was so short that it really would not serve as a hose barb. I realize that I could have most likely found some sort of fitting to screw on there to conver to a hose barb to allow me to all my own hose and base, but I had a gift certificate to my LBS and just used that to pick up a new pump.
