That's not "tall gearing", that is about 1:1, it looks like. I think that arrangement is pretty normal for larger trikes like that. They aren't intended to ride to the next county or anything.
Even 1:1, though, for moving an 882 pound load, can cause TREMENDOUS knee damage if there's the slightest incline.
I'm thinking, maybe a 16T (can be done with a Mountain Tamer Quad adapter - or, they're using motorcycle chain, they're probably doing custom sprockets anyway) chainring, and a 34T sprocket? That'd give somewhere around 12 gear inches, if I'm right about that being 559.
Or just use a mid-drive.
