I have 8S Ultegra bar ends on my touring bike that do this. It has nothing to do with indexing a gear as the feel is very slight, but it happens on a downshift from the big to middle chainring. I was told to rotate the adapter 90 degrees, which I dutifully did, but the 'bump' in the lever is the same. It wouldn't be felt shifting a double.
