I don't think it is practical to convert that to an index system. I suspect whatever you do to get housing stops and get the cable running into the holes will cause too much friction. The only practical way to do it is to use a clamp on cable stop, like problem solvers. But then you also need bolt on cable guides.

That looks like a nice frame, if you can't enjoy it with that great escargot style setup pass it along and get something more conventional for your brifters.
