One of the reasons this arrangement disappeared with the introduction of index shifting mid 80's is the issue I stated (and resolved) about keeping it all in adjustment in my earlier post.

As far as routing cable into the DT, I'd probably run a clamp-on double cable stop right in front of the "escargot". But it's not going to be real elegant at this entry point.
