2 possibilities come to mind;
stem is not inserted enough
stem is inserted too far

more likely the first case I'd say
on the side of the quill stem there should be markings -look close through the paint
this is the minimum safe insertion marking
the stem must be inserted into the steerer tube far enough to cover this marking
otherwise, insecure attachment, death, etc

I did insert it past the minimum marking on the stem. Read on another thread here about putting some light oil to help the components function better. So I used a minimum amount of synthetic silicone grease.

Funny you mention weak hands. I exerted a bit more force to tighten up the bolt. The fork seems to be a bit more difficult to get out of alignment. with the bars. Will see tomorrow how it actually is on a ride.
