Just a little update several months after I started the thread. I'm up to about an hour and a half at an 85 cadence at a decent resistance with higher gears. The trainer says I've gone ~ 22 miles =). I can do 100 cadence for a while without my feet feeling like they'll fly off my legs.

I'm doing my first metric century this weekend. I expect I'll be able to finish it if the bursitis on my elbow doesn't cause me any problems.
