I was in a rush when I posted before, and meant to say fulcrum lever , not fulcrum clip. The lever attaches to the axle, and has a barrel adjuster on it, so it gives you another adjustment point. I think it's the best option for small frames.
For small hands, the thumbie shifter works best I think. At least it works well for my 7 year old granddaughter.
Ah, that's the part for which I couldn't figure out the name. Unfortunately, nobody seems to stock them stateside, but it really does look like the ideal solution.
