Some things to check (aside from the limit screws and FD height, which you already checked):

1) Bent tooth on your big chainring
2) Bent front derailleur cage
3) Protruding pin on one of your chain links
4) Chain slightly too long (RD can't supply enough tension in small chainring, lower half of cassette combo)
5) "Sticky" chain link
