Yeah, this happened to me once....WHILE I WAS RIDING!!

Luckily it was the rear wheel and I was going slowly. It was an old Mavic CD4 that was worn way too thin from the brakes. I regret not taking a pic of it because the section was larger than yours; about 8 inches.
