I had this problem with my new Egg Beaters and Pearl-Iszumi road shoes. Thought the holes were smaller than the cleat bolts. Actually, they had small lugs screwed in, I assume, to protect the threads. I just needed a small (tiny) allen wrench to remove them and the cleat bolts could go in.
