Cyclocross style "in-line" levers might be just what you're looking for. The Park Tools tech website shows how they're installed, but not connected to center- pulls, though it's the same installation of brake line once you get past the 2nd lever. Not exactly c&v, but they work pretty well if required. Hope this helps!
