Let me add a third... is any training really required for those folks that live and work in small towns that do not have any form of heavy traffic?

Towns like Chicago, Houston, NYC, San Diego, LA and others have fast dense traffic on their roads; cyclists have to deal with that. Towns like Granbury Texas, and perhaps Burlington Iowa, and Ringsted Iowa, and Langley Washington... do they need any special training? Are cyclists there doing just fine without VC or EC or whatever... is this possible? Maybe in these small towns everyone moves at the pace of a cyclist anyway.
