I was just there a few weeks ago, through a very last minute deal, self guided, with a tour company. In fact they published my feedback on their blog:
http://newblog.biketoursdirect.com/?p=826 My rental bike came from Florence by Bike but I had to take what they had available with such short notice and it wasn't ideal - a triple ring setup willier. But it wasn't their fault, and I made the best out of it. The extra rolling resistance served for training.
I rode by a nice shop near Colle de Valle D'Elsa named Gippo Bikes which rented some very nice bikes. I'd definitely call them.
