My problem was the rear wheel quick release (light weight type) didn't have enough clamping force. Everyone whoever heard the click said it was the bottom bracket including some LBS wrenches. It seems like the hollow chain stays (carbon bike) amplified the click to a point it sounded like it came from the BB.

After several BB remove, lube, re-installs the noise didn't change. I also never connected the dots to how the noise frequency would change after removing and installing the rear wheel; it never went away but it would be more intermittent and/or quieter then increase in frequency and/or volume after some hard climbs. When I installed the light weight quick releases I had lightly greased everything which made it even harder to think about the quick release as the source of the noise. Installing my Wheel MFGs OEM quick release fixed the problem but the release clamp pressure still has to be higher than I would consider normal.
