Bikes: '96 Litespeed Catalyst, '05 Litespeed Firenze, '06 Litespeed Tuscany, '20 Surly Midnight Special, All are 3x10. It is hilly around here!

You are missing the bearing balls or cartridge, the top race and the centering cone. Frankly, I recommend going to a bike shop and starting with a brand new complete headset.
