Could be a bad cotter-pin, as sailorbenjamin mentions. Your description sounds very similar to my experience when I had a loose cotter-pin in an old Sears 3 speed. Just one side, once a rotation, a sort of stutter/click, like one crank arm wasn't quite solid. (Which would be addressed during BB overhaul, as rhm suggested)
