The poster does not give the brand/model of the bike, how he/she rides it, etc. etc. etc.
I'll comfortably stick with my thoughts/opinions that this should not happen on a one year old bike...I'll add that this person just does not seem like someone that rides a lot.
It doesn't matter how the OP rides a bike or how often. And in fact, the age of the bike is irrelevant.

The LBS seems, on the evidence presented by the OP, to have failed to do a complete job to ensure the bike was safe to ride, as it might be ridden by someone irrespective of how advanced their knowledge and skills might be. This was not a free service, and the LBS had an obligation to ensure their replacement of the chain did not impact on other operational aspects of the bike. Their failure to see through this obligation, resulted in the OP being quite badly injured.

I suppose an analogy would be a car mechanic replacing three of the four sets of brake pads on a vehicle, the fourth set being down to the metal The owner would be entitled to think the job had been done properly, but ends up injured when the car crashes as a result of pulling to one side under hard braking 100 yards after pulling out of the mechanic's driveway.
