Google maps will give you visual and spoken turn by turn directions. "Custom" is a little more complicated but I think you can google "custom routes in google maps" for ideas.

Remember that battery life is seriously affected by GPS always on. Also smartphone displays aren't so easily seen in bright sunlight.
